Create new supplementary form

You can create a new supplementary form from the OSM Home page or from the library page.
Create from Library page

On the Library page, click the Add
button.

On the Create new supplementary form screen, enter the details. The fields with a blue star indicate required information.

Click Create new supplementary form.
