Create new supplementary form

You can create a new supplementary form from the OSM Home page or from the library page.
Create from Library page

On the Supplementary Form Library page, click the Add
button.

On the Create new supplementary form page, enter the necessary details. The fields with a blue star indicate required information.
For more information, see Fields on Supplementary Form information header.

Click Create new supplementary form. You are returned to the details page of the supplementary form, not to the library.
