• Title*

The title of the supplementary form. This field is required.

• Number

The number will be auto-generated.

• Description

A description of the collection.

• Site*

The site where the collection will be used. This field is required.

• Responsible

The name of the person creating the collection. Select from the drop-down list, or click Swap to free text to enter a name.

• Valid from

A start date that the collection will be valid.

• Valid to

An end date that the collection will be valid.
