Util_Log

Remove "wo_id", "oper_id", "seq_no", "item_id, and "step_no" fields. The first 4 will be replaced by the same fields in the new job_util_log_link table. Add a unique key on event_time, ent_id.

Add "raw_reas_cd" (string40, nullable) before "last_edit_comment" to record the underlying raw reason code for each record, if it was limited by a value from Factory Connector.
