SPC_Char_Job

Remove the "form_name" field since the form table it needs to be useful is being removed.

Add inhibit-delete foreign key from "assoc_file_type" to doc_type.

Rename "last_modified_by" to "last_edit_by" and "last_modified_at" to "last_edit_at". Move "created_by" between these two fields and "created_at" after them.
