Before uploading
Before uploading the PDF form, ensure you have completed the steps in: Preconfigure the PDF form in Adobe
Upload the PDF file into Work Permit Form Editor

Click the Upload button in far right top corner.

Browse to select the PDF file and click Open.

When you upload the PDF form, the pre-configured form fields may not appear right away. Scroll to the bottom of the PDF to fetch the fields. After a few seconds, the form will refresh itself and automatically save.

When you have finished configuring fields in Form Editor, click Save and then click Return to OSM to go to the web application.
