Type and site parameters

• Site

Select the Site.

• Type

Select the form type, as defined in Add supplementary form type definitions.

• Active

You can not activate the type for Site until a form is uploaded.

• Default

Select the checkbox if this type/site is the default.

• Subtype is required

Select the checkbox if this type requires subtypes. See Require subtypes.

• Prog value

This field is for advanced programming values.

• Name of form

The name of the Supplementary form appears, after a form has been uploaded.

• Filename

The filename of the Supplementary form appears, after a form has been uploaded.

• Description

You can add a description of the Supplementary form. This is optional.
