Configure Supplementary forms

After you have created the custom HTML5 forms, you can configure the form definitions in OSM Administration and upload the forms to the system.

• Add supplementary form type definitions

• Associate types to sites and upload form

• Type and site parameters

• Reupload supplementary form

• Show sites for a type

• Add subtypes for supplementary form

• Require subtypes

• Include attachment on supplementary form
