Add Form Fields in Adobe

Open your PDF form in Adobe Acrobat.

On the Tools tab of Acrobat, click the Prepare Form tool.

Use the tools in the Prepare Form toolbar to add different types of form fields to your PDF form.

Give each field a name that starts with the prefix WPFORM_.

The names that you give to the form fields will be used in OSM. They should only contain:

Uppercase letters (A to Z)

Numbers (0 to 9)

The underscore character (_)
Important: The period character (.) and space character ( ) cannot be used in form field names.

Set the properties on each field.
Recommendation for General tab
We recommend that the Read Only checkbox is always selected, regardless of type of field and whether the field should be read-only or not in the Work Permit Form Editor itself.

Recommendation for Appearance tab
If you are copying text into your form that contains unusual characters, they will not display or print correctly due to being unsupported characters. To avoid this issue, we recommend specifying Arial on the Appearance tab, which is a Unicode-supporting font and the preferred font to use for OSM.
If an unsupported font is selected in the Font field, it can produce unreliable results.

Continue until you have defined the type, name, and properties of all fields on the PDF form.

Save the form.
Reduce PDF file size
For the best performance, you should reduce the file size of PDF as much as possible.

In Adobe, on the File menu, point to Save as Other, then click Reduced Size PDF.
